Staff Packet

 

Welcome to Camp Olympia’s Staff for 2010! You are about to embark on what we are sure will be one of the most O-Mazing experiences of your life. We have included everything we think you will need in order to help you prepare to have the best summer ever! This counselor “packet” lists all of your paperwork that is required to have on file for you as well as what you will need to pack. So let’s get started…..

WHEN TO ARRIVE for Counselor Training School (CTS) Remember it’s not only mandatory but it’s a BLAST!
Returning Staff – Plan to arrive by 5:00pm Sunday, May 30th
New Counselor – Plan to arrive at Olympia between 1:00 – 2:00pm on Monday, May 31. If you will be traveling a long distance and need to come in Sunday May 30 that will be fine, please make arrangements with Cody and/or Michelle.
Flying In – If you will be arriving by plane, please plan to make your flights for Sunday, May 30 into Houston Intercontinental Airport (IAH) and book your flight times with David Stubblefield.

 

ALL PAPERWORD REQUIRED TO WORK Please download and fill out these forms and mail them back to us no later than May 15.

I-9 FORM http://www.uscis.gov/files/form/i-9.pdf

· Complete the blank I-9 form.
- When you come to part 2 on the form, look over list A, B, & C. Please check the box(es), one in column A or one in columns B & C denoting the documents you will be providing as proof of citizenship. Please submit photocopied documents (social security card, birth certificate, drivers license, passport, etc..) to be placed in your personnel file.

- You must have these documents along with this correctly completed form turned in prior to orientation. Annie Peter will verify this form along with your photocopied documents. Please come prepared to insure the process will run smoothly. Thank you!

· Everyone must bring a completed W-4 form to camp. Please consult your parents if you are uncertain as to the number of exemptions to claim.

Please read the following before completing your W-4 form.

The address that you place on this form will be used to mail your official tax form next year. Please notify us immediately if your address changes.

 

 WORKER'S COMP INSURANCE

· Everyone must read through the Employee Notice of Network Requirements and sign the back form and send it in with all paperwork.

 MEDICAL FORM

· You must have your medical form completed by your physician before your arrival at Camp Olympia. Your completed medical form may be turned in no later than orientation May 31.

· Please fill this form out whether you will be bringing your bike, skateboard or long board with you or not, as this is requred by our insurance.

 

CERTIFICATIONS

· Please make front and back copies of any current certifications you hold. Examples: WSI, LGI, LT, FA, CPR, etc...

· If you are interested in becoming a certified Lifeguard contact David Stubblefield. Click here for more details.

· Lifeguard Recertification will be offered Saturday, June 5 at 1:00pm - contact David Stubblefield to sign up by May 15.
