· Complete the blank I-9 form. - You must have these documents along with this correctly completed form turned in prior to orientation. Annie Peter will verify this form along with your photocopied documents. Please come prepared to insure the process will run smoothly. Thank you!
W-4 FORM http://www.irs.gov/pub/irs-pdf/fw4.pdf
· Everyone must bring a completed W-4 form to camp. Please consult your parents if you are uncertain as to the number of exemptions to claim.
Please read the following before completing your W-4 form.
The address that you place on this form will be used to mail your official tax form next year. Please notify us immediately if your address changes.
