STAFF PACKET

Welcome to Camp Olympia's Staff for 2011!  You are about to embark on what we are sure will be one of the most O-Mazing experiences of your life.  We have included everything we think you will need in order to help you prepare to have the best summer ever!  This counselor "packet" lists all of your paperwork that is required to have on file for you as well as what you will need to pack.  So let's get started....

WHEN TO ARRIVE for COUNSELOR TRAINING SCHOOL (CTS) - Remember it's not only mandatory, but it will be a BLAST!

Returning Staff - Plan to arrive by 5:00pm Saturday, May 28th

New Staff - Plan to arrive at Olympia between 1:00 - 2:00pm on Sunday, May 29.  If you will be traveling a long distance and need to come in Saturday, May 28 that will be fine.  Please make arrangements with Cody and/or Michelle.

Flying In - If you will be arriving by plane, please plan to make your flights for Saturday, May 28 into Houston Intercontinental Airport (IAH) and book your flight times with Michelle Mauldin.

ALL PAPERWORK REQUIRED TO WORK - Go to the Staff Login screen and follow these two steps to complete all forms necessary to work at camp.  Complete all online forms and paper-based forms by May 15.  (All paper forms need to be completed and mailed to camp (723 Olympia Dr. Trinity, TX 75862), faxed to camp at 936-594-8143  or e-mailed to apeter@campolympia.com.)

STEP 1:

  • Login to your staff account using the information emailed to you to access the "forms dashboard", where the necessary forms are listed.
  • Complete the following online forms:
    • Counselor Personal Information Sheet - complete so we can learn a little bit more about you AND how to handle any medical charges you may incur.
    • Health Form - You must have your medical form completed by your physician before your arrival at Camp Olympia.  Your completed medical form may be turned in no later than the start of orientation, May 29.
    • Helmet Assumption of Risk - Everyone must complete this helmet assumption of risk form, whether or not you will be bringing your bike, skateboard or long board, as this is required by our insurance.
    •  Workers Compensation Acknowledgement - Complete after reading the Worker’s Comp Insurance - Employee Notice of Network Requirements

STEP 2: 

Download the following forms found on your forms dashboard, print, complete and send them in to camp by May 15. 

  • I-9 FORM
    • Complete the I-9 form.  You must complete and sign Section 1.  Please note that Camp Olympia will complete Section 2 & 3. 
    • On the back of the form (page 5 of the linked file above), look over list A, B & C.  Please circle the box(es), one in Column A or one in columns B & C denoting the documents you will be providing as proof as citizenship.  Please bring original documents (such as, social security card, birth certificate, driver's license, passport, etc.) with you to camp.  Please come prepared to insure the process will run smoothly.  Thank you! 
  • W-4 FORM
    • Complete the W-4 form.
      • Please consult your parents if you are uncertain of the number of exemptions to claim.
      • The address that you place on this form will be used to mail your official tax form next year so please enter your permanent mailing address.  Please notify us immediately if your address changes.
  • W-11 FORM
    • W-11 HIRE Act - If you have worked less than 40 hours in the 60 days preceding hiring, please complete this W-11 form and bring it to camp with you. 
  • ASSUMPTION OF RISK FORM - Please read and sign the Staff Permission to Attend, Release and Indemnity form and send it in with your paperwork.
  • PARENT INSURANCE CARDS - Send in copies of your health insurance cards. 
  • PHYSICIAN'S EXAMINATION - Take the examination page (from the online medical form) with you for the doctor to complete for your physical.


CERTIFICATIONS

     - Please make front and back copies of any current certifications you hold.  Examples: WSI, LGI, LT, FA, CPR, etc.

     - If you are interested in becoming a Certified Lifeguard, contact Chad Peter.  Click here for more details.

     - Lifeguard Recertification will be offered Friday, June 3 at 1:00pm.  Contact Chad Peter to sign up by May 15.  Click here for more details.
